Getting Started

Who should I call to get started?

We’re so excited to meet you! If you’re interested in assisted living in Independence, Missouri, then please contact our Admissions team at (816) 795-1433 to discuss your next steps over the phone. We’d love to get to know you better, answer any questions you might have, and have the opportunity to introduce you to a few members of our team!

Is there a waiting list for admission?

Availability can vary depending on current occupancy and individual care needs. We recommend contacting our admissions team directly to discuss availability, schedule a tour, and learn more about the admission process.

Do you have short-term and long-term care?

Yes, the Villages of Jackson Creek provides both short-term and long-term care options. Short-term stays include post-hospital rehabilitation, step-down care, and respite services, while long-term care is available for individuals who require ongoing medical support. All care is provided within our Independence facility.

Can I bring personal belongings?

Yes, residents are encouraged to bring personal belongings such as photos, decor, and other meaningful items to make their space feel more comfortable and familiar during their stay.

What insurances do you accept?

The Villages of Jackson Creek accepts a variety of payment options, including private pay, Medicare, Medicaid, and many PPO and HMO insurance plans. Our admissions team will work closely with you to review your coverage and help you understand your benefits before you begin care.

Will Medicare cover my stay at the facility?

Residents must have a Medicare card that reads “Hospital Insurance.” They must also be admitted to the facility with a physician order within 30 days of a three consecutive night hospital stay. Generally, during the first 20 days of care, Medicare will pay 100%. For days 21-100, a daily co-insurance co-pay is required. Medicare does not cover skilled nursing fees over 100 days.

Who do I talk to about questions in regard to financials, billing, and/or admission paperwork?

We employ a full-time Business Office Manager for all financial and billing concerns. Should you have any questions regarding these matters please make an appointment to see our Business Office Manager by contacting the front desk/receptionist.

The Admissions Coordinator will guide you through the necessary forms required on admission and review the rules and regulations regarding Medicare, Medicaid, and various insurances.

Healthcare Services

What services are offered at your skilled nursing facility?

Our Independence facility provides a wide range of clinical services to support both short-term recovery and long-term care. In addition to 24/7 skilled nursing and medication management, we offer IV therapy, wound care, pain management, enteral feeding, diabetic management, and post-hospital extended care. We also provide specialized support such as catheter care, colostomy and ileostomy care, dementia care, and hospice or respite services when needed.

What type of nurses do you have on staff?

Our nursing team includes licensed professionals who provide care 24 hours a day. Care is overseen by our Director of Nursing and coordinated with our on-site Medical Director to ensure consistent, high-quality clinical support. Our staff also participates in ongoing training and education programs, helping maintain strong standards of care across our facility.

What other professionals do you have on staff?

In addition to our nursing staff, we offer a full interdisciplinary team to support resident care. This includes physical, occupational, and speech therapists, a registered dietitian, and social services professionals. We also provide access to specialists such as psychologists, audiology services, and other providers as needed to support comprehensive care.

What kind of medical needs can you accommodate?

We are equipped to care for a wide range of health needs, including post-surgical recovery, chronic illness management, wound care, diabetes, mobility limitations, and stroke rehabilitation. Each resident receives an individualized care plan tailored to their condition, goals, and level of support needed.

When will the doctor see me?

Doctors typically visit within 24 hours of admission. Your nurse will communicate any medical concerns with your doctor before and after they see you. 

What type of rehabilitation is available on site?

Our on-site rehabilitation program includes physical, occupational, and speech therapy designed to help residents regain strength, mobility, and independence. Therapy is supported by dedicated treatment spaces and equipment focused on balance, movement, and functional recovery. Our restorative nursing program also helps residents maintain progress outside of formal therapy sessions.

Do I need to bring my medications?

No, you do not need to provide medications unless specifically told to do so. Upon admission, we will order your medications using our off-site pharmacy, then securely store them on-site to ensure safety. 

How long is a typical stay?

Each case is different and unique. Patient progress is reviewed weekly by our Interdisciplinary Team (IDT) of nurses, therapists, and dietitians. Our main goal is that you reach your highest level of independence.

Amenities

What amenities are offered at your skilled nursing facility?

Residents at the Villages of Jackson Creek enjoy a comfortable and supportive environment with amenities designed for everyday living. Our community offers private and semi-private rooms, accessible bathrooms with walk-in showers, daily housekeeping and laundry services, cable TV, and three meals served daily with snacks available. Residents also have access to engaging activities, social programs, and an on-site beauty shop available weekly.

What is your activities program like?

We strive to create social programs and individualized activities to match the capabilities and needs of each resident. This also has a powerful impact on the healing process of those residents involved in some type of rehabilitation. Specific activities include: active resident councils, musical performances, current events update, reading the newspaper, singing exercises, fitness activities, stretching exercises, religious meetings, outside entertainment, games, trivia, history, and volunteer involvement. We also strongly encourage families to participate with the residents in various activities.

A monthly calendar is posted to keep residents informed of upcoming events such as birthday parties, movies, musical programs, outings, candlelight dinners, ice cream socials, and other special events. We always welcome friends and family to visit and attend our special events.

What is your dining program like?

Upon admission, our Registered Dietitian and/or Food Service Director will interview your loved one regarding special needs with regards to food preferences. Our own food service personnel prepare all the meals. Special care is taken to serve flavorful and appetizing foods. All menus are developed according to prescribed medical diets ordered by the physicians.

Should you wish for another food item on a particular day’s menu, please make the CNA aware of your request. This information will be passed on to the Food Service Director and they will try their best to accommodate needs.

Our Dietitian and Food Service Director also monitor nutritional status using monthly weights, daily intake records, and relevant lab data. The results of these assessments are discussed at care conferences.

Is there any private space available to use?

You may reserve the activities lounge for any special events (birthday, holidays, meals) with notice.

Policies

What is your visitor policy?

We recommend visiting from 8 am until 8 pm, after that time we lock our doors as most residents are asleep. Please call in advance for after-hours visits to ensure access.

Is there a limit to the number of people who can visit at one time?

Although we do not have a formal policy stating visitation guidelines, we suggest using the common areas of the facility to meet with your loved one. If it is not possible to visit in one of the common areas, we suggest a limit of one to three people in a resident room at one time.

Are pets allowed to visit?

We would love for your family pet to visit. Please remember to clean up after them and take the proper safety measures at all times. All pets are to be kept on a leash and should have all current vaccinations.

Is smoking allowed?

For the safety and welfare of all residents and staff, we are a non-smoking facility. There are designated smoking areas in outdoor areas.

Can I take my loved one home for a visit?

Residents may have a leave of absence only with a physician’s authorization, which can be arranged through the nursing staff. Upon arrival and departure, the resident or responsible party must sign the LOA (leave of absence) book, located at the nursing station.

Questions & Concerns

How do you protect patients, staff, and visitors from COVID-19 and other infectious diseases?

Our team is highly trained and experienced in caring for high-acuity patients and infectious diseases like COVID-19. Our dedicated infection prevention nurse helps ensure we follow current infection prevention guidelines and best practices. This includes educating all staff about the appropriate use of personal protective equipment (PPE) such as N95 masks, surgical masks, gloves, and gowns and when to use COVID-19 protocols required by local department of public health and/or the CDC.

What do we do if we have personnel concerns?

Our primary concern is that your loved one feels comfortable here and that their needs are being met. You should feel secure and at ease with our staff, particularly those providing direct care. If you have any concerns or praise for our staff, please feel free to contact our social services department. They will be able to address any apprehensions you may have regarding any particular staff members.
